Wisdom Well Retreats, LLC Therapist Retreat Participation Agreement Nature of Services Provided We are happy to have you participate in the [retreat name]. This retreat offers [CE credits] Continuing Education (CE) credits, waiting approval from NBCC and ASWB. It is the participant's responsibility to verify that CE credits offered are appropriate for their licensure and to confirm acceptance with their licensing board. Wisdom Well Retreats, LLC makes no guarantee that any licensing board will accept the CE credits offered. In addition to CE training, this retreat includes a [days] day, [nights] night, all-inclusive experience consisting of: accommodations; meals and non-alcoholic beverages; wellness activities; guided group tours and excursions. While Wisdom Well Retreats, LLC is a licensed mental health practice, participation in this retreat does not constitute therapy, psychotherapy, counseling, or healthcare services, and is not a substitute for professional healthcare. Should a participant experience a mental, emotional, or physical condition requiring care, referral resources can be provided upon request. Professional Privacy & No Clinical Relationship We strive to respect participant privacy at all times. Legal rules of healthcare confidentiality (e.g. HIPAA) do not apply to this retreat. Participation does not establish a clinician-client, supervisory, or consultative relationship between Wisdom Well Retreats, LLC (including its facilitators, presenters, and staff) and any participant, and rules of privilege do not apply. Facilitators are not acting as participants' therapist, clinical supervisor, or consultant during this retreat, and no mandated-reporting duty, supervisory liability, or clinical documentation obligation is created between Wisdom Well Retreats, LLC and any participant by virtue of attendance. Peer Confidentiality & Code of Conduct Because this retreat brings together licensed mental health professionals who may share personal or clinical experiences during trainings, workshops, or informal group settings, all participants agree to treat information shared by other participants as confidential and not to disclose, discuss outside the retreat, or use such information for any purpose without the sharing participant's consent. This is a peer expectation of confidentiality, not a legal privilege. Information shared among participants is not protected by therapist-client privilege, HIPAA, or other statutory confidentiality protections. Participants remain bound by their own professional code of ethics (e.g., APA, ACA, NASW, AAMFT, or applicable state board rules) in all interactions during the retreat. No solicitation of clients, supervisees, or business is permitted during the retreat unless approved in advance by Wisdom Well Retreats, LLC. Financial Requirements A [deposit] USD non-refundable deposit is required to secure your spot. Monthly payment plans are available. Remaining balances are divided equally across the months remaining until the final payment due date. Payments are charged automatically each month on the monthly anniversary of your booking date, with the balance divided equally across the months remaining until the final payment, which completes no later than 45 days before the retreat start (60 days for international retreats). Cancellation Policy The deposit is non-refundable under all circumstances. Refunds apply only if the balance has been paid in full at time of cancellation: 50% if cancelled 60 or more days prior; 25% if cancelled 30 to 59 days prior; no refund within 29 days of the retreat start. Participants on a payment plan waive refund eligibility unless the balance is paid in full prior to cancellation. If the balance has been paid in full, the option to re-schedule your attendance may be provided and considered. If Wisdom Well Retreats, LLC must cancel the retreat due to unforeseen circumstances (including but not limited to natural disaster, political instability, pandemic, or vendor failure), a full refund of amounts paid directly to Wisdom Well Retreats, LLC will be issued. Wisdom Well Retreats, LLC is not responsible for airfare, travel insurance premiums, or other incidental travel-related expenses. Terms & Conditions Grievances regarding the retreat experience must be submitted in writing to info@wisdomwellretreats.com will be addressed within 10 business days. This internal grievance process is separate from, and does not replace, a participant's right to file a complaint with their professional licensing board or a CE-approving body regarding CE content or credit. Audio and/or visual recording of trainings is prohibited without advance written consent, in order to protect the privacy of all participants and presenters. Health, Safety & Emergency Protocol Participants must disclose any medical, physical, or mental health conditions relevant to their safe participation prior to the retreat, and are responsible for consulting their own physician regarding fitness to travel and participate in physical or emotionally intensive activities. In the event a participant experiences a medical or mental health crisis during the retreat, Wisdom Well Retreats, LLC staff will assist in connecting the participant with local emergency services and/or referral resources, but Wisdom Well Retreats, LLC is not acting as, and does not assume the role of, the participant's treating clinician in doing so. All participants and guests are required to obtain travel insurance, including coverage for medical evacuation where applicable, and to provide proof of coverage prior to attending the retreat. Acceptance of Risk & Waiver of Liability I voluntarily consent to my participation in the retreat and understand that travel, physical activities, and group emotional-processing exercises involve inherent risks, including injury, illness, or death. I acknowledge that it is my responsibility to consult a physician prior to participation and to disclose any relevant medical conditions. I agree to indemnify and hold harmless Wisdom Well Retreats, LLC, its affiliates, contractors, facilitators, and representatives from any claims arising from my participation, except in cases of willful misconduct or gross negligence. Participants may be excluded from retreat activities if behavior, concealed health issues, or disruptions endanger themselves or others. Removal does not entitle the participant to a refund. Consent for Photography & Video I authorize Wisdom Well Retreats, LLC to make photographic and/or video recordings of my participation for marketing, advertising, and educational purposes. This consent may be revoked in writing at any time. Governing Law & Dispute Resolution This Agreement shall be governed by the laws of the State of Washington without regard to conflict-of-law principles. Any dispute arising from this Agreement or the retreat shall first be submitted to good-faith mediation before either party pursues arbitration or litigation, except that Wisdom Well Retreats, LLC may seek injunctive relief where necessary to prevent immediate harm. Severability & Entire Agreement If any provision of this Agreement is found unenforceable, the remaining provisions shall remain in full force and effect. This Agreement, together with any signed addenda, constitutes the entire agreement between the participant and Wisdom Well Retreats, LLC regarding the retreat, superseding any prior oral or written understanding. Acknowledgment & Agreement By signing below, I confirm that I have read, fully understand, and voluntarily agree to all terms, conditions, financial obligations, cancellation policies, confidentiality expectations, and participation requirements outlined above.
